CHUKAI: Questions abound as to how and why the two Royal Malaysian Air Force (RMAF) Hawk 108 pilots died last Thursday in a swamp in Chukai, despite ejecting and having their parachutes deployed.


A probe into the crash by an investigation team, led by Lieutenant-Colonel John Sham, is proving difficult.

Virtually none of the nation’s combat aircraft, unlike most new-generation fighters, carried a black box, which housed a flight data recorder, cockpit voice recorder and crash-survivable memory unit.

Despite the availability of state-of-the-art equipment, powerful radar and tracking satellites, it would take investigators months to determine what transpired.

With the full details of investigations rarely made public by the military, several veteran military fighter pilots and aviation medicine practitioners, speaking on condition of anonymity, gave their views on why combat jets kept crashing.

Major Yazmi Mohamed Yusof, 39, and Major Mohd Hasri Zahari, 31, were killed in the crash.

Both had at least a decade of flying experience under their belts.

The pilots, who were familiar with conducting test flights of newly-serviced aircraft like the Hawk 108, were said to have taken great risks when performing complex manoeuvres before certifying the aircraft fit for combat duty.

“Any fault — either human, mechanical, atmospheric or even sabotage — could have caused dire consequences, even in non-dogfight procedures,” the sources said.

Some argue that both pilots could have lost consciousness when ejecting from the jet fighter at up to 14g (14 times the force of gravity) during a high-speed manoeuvre.

Other factors that could lead to accidents included loss of situational awareness and flight proficiency, disorientation, task misappropriation, loss of flight vision at blind spots, failure of onboard systems, avionics and instruments, and airframe structural defects.

Yazmi was tasked with test-flying the jet after it had spent months in the maintenance hangar for servicing.

A less-senior pilot based in Butterworth was supposed to test the aircraft, but Yazmi, as the 6th Squadron’s commanding officer, had undertaken the responsibility as he was available to certify its airworthiness certificate.

Yazmi had performed a ground test of the aircraft and was scheduled to perform the aerial test with co-pilot Hasri for 45 minutes.

Being an experienced pilot with the Hawks, Yazmi might have attempted to save the 21-year-old aircraft.

An air examiner familiar with such accidents said this might be the reason the aircraft sustained minimal damage and remained intact upon crashing, while the pilots appeared to have bailed out at the 11th hour.

DUA kerusi lenting dan beberapa serpihan pesawat Hawk 108 antara penemuan terbaru dalam operasi berhampiran Kampung Yak Yah, Chukai, Terengganu.

Markas Tentera Udara dalam kenyataannya hari ini berkata, TUDM akan meneruskan operasi mencari serpihan pesawat dalam usaha melengkapkan siasatan bagi menentukan punca kejadian.

Menurutnya, keadaan muka bumi dan tanah lembut menyukarkan operasi.

Bagaimanapun katanya, TUDM mendapat bantuan daripada beberapa agensi dalam usaha mencari dan mengumpulkan serpihan pesawat.

"Antara agensi yang terbabit ialah Tentera Darat Malaysia, Rejimen Askar Jurutera Diraja, polis dan Institut Penyelidikan Sains dan Teknologi Pertahanan (STRIDE)," katanya.

Dalam perkembangan berasingan, TUDM menafikan dakwaan pesawat Hawk 108 sebagai bermasalah kerana ia sudah melalui proses penerbangan pemeriksaan pasca senggaraan dua hari sebelum nahas.

Katanya, TUDM tidak berkompromi mengenai keselamatan pesawat dan sentiasa memberikan keutamaan ketika penyelenggaraan yang dilakukan di bawah kawalan TUDM.

Dalam kejadian 11.30 pagi Khamis, pesawat pejuang Hawk 108 dinaiki Mejar Yazmi Mohamed Yusof, 39, dan Mejar Mohd Hasri Zahari, 31, terputus hubungan selepas 30 minit berlepas dari Pangkalan Udara Kuantan, Pahang.

Operasi SAR dijalankan menemui kedua-dua mangsa di Chukai, Kemaman, Terengganu pada 2.30 petang hari kejadian.

KUALA LUMPUR: Dua kerusi anjal dan beberapa serpihan pesawat Hawk 108 M40-02 milik Tentera Udara Diraja Malaysia (TUDM) yang terhempas berhampiran Kampung Yak Yah di Chukai, Terengganu, Khamis lalu sudah ditemui.

TUDM menerusi kenyataan media dikeluarkan Markas Tentera Udara, sebentar tadi memaklumkan operasi mencari serpihan pesawat masih diteruskan dalam usaha melengkapkan siasatan berhubung punca kejadian.

"Keadaan muka bumi dan tanah yang lembut serta berpaya di tapak kejadian menyukarkan operasi ini.

"Bagaimanapun, TUDM sudah mendapatkan bantuan daripada beberapa agensi bagi melancarkan usaha pencarian dan pengumpulan serpihan termasuk Tentera Darat, Rejimen Askar Jurutera Diraja, polis dan Institut Penyelidikan Sains dan Teknologi Pertahanan (STRIDE)," katanya.

Menurut kenyataan itu lagi, TUDM menyangkal khabar angin yang viral menyatakan pesawat terbabit bermasalah sehingga menyebabkannya terhempas.

"Pesawat Hawk 108 terbabit sudah melalui proses penerbangan pemeriksaan pasca selenggaraan dua hari sebelum nahas itu.

"TUDM tidak akan berkompromi mengenai keselamatan dan 'airworthiness' (kelayakan terbang) pesawat serta sentiasa menekankan tahap tertinggi dalam penyelenggaraan pesawat," tambah kenyataan itu lagi.

Jumaat lalu, pasukan mencari dan menyelamat (SAR) menemui kedudukan pesawat latihan lanjutan dipandu Mejar Yazmi Mohamed Yusof, 39, dan Mejar Mohd Hasri Zahari, 31, yang terhempas setengah jam selepas berlepas dari Pangkalan Udara Kuantan pada jam 11, Khamis lalu.

Sementara itu, Panglima TUDM, Jeneral Datuk Seri Affendi Buang bersama beberapa pegawai kanan termasuk Inspektorat Jeneral TUDM, Mejar Jeneral Datuk Johar Razali Ahmad melawat lokasi utama pesawat itu terhempas bersama pasukan Lembaga Penyiasatan diketuai Leftenan Kolonel A John Sham, pagi tadi.

KEMAMAN 19 Jun Radar pengesan Tentera Udara Diraja Malaysia (TUDM) merakam penuh tragedi nahas pesawat Hawk 108 sehingga ke saat akhir ia terhempas, kata Panglima TUDM, Jeneral Datuk Seri Affendi Buang.

Beliau berkata, radar pengesan itu digunakan pada hari penerbangan pesawat Hawk pada Khamis lepas, sebelum nahas itu berlaku.

"Dari segi aspek penyiasatan, TUDM telah memperoleh bukti awal daripada radar pengesan dan masih terlalu awal untuk kami membuat kesimpulan dengan apa yang sebenarnya berlaku.

"Di lokasi nahas, TUDM berjaya menjumpai kedua-dua kerusi anjal pesawat Hawk dan bahagian besar bangkai pesawat itu yang tidak tertimbus di kawasan paya," katanya pada sidang akhbar ketika meninjau kawasan nahas pesawat Hawk di Kampung Yak Yah di sini, hari ini.

Affendi berkata, selepas memperoleh komponen pesawat itu, TUDM membuat lakaran kasar untuk menentukan kemungkinan keadaan sebenar pesawat itu terhempas di kawasan berkenaan. - BERNAMA

Jelas beliau, bagi memastikan siasatan itu dijalankan dengan sistemaik dan profesional, TUDM meminta kerjasama Institut Penyelidikan Sains dan Teknologi Pertahanan (STRIDE) dan pihak pembuat pesawat Hawk bagi mendapatkan gambaran sebenar punca kejadian.

Ditanya sama ada pesawat itu telah menjalani pemeriksaan sebelum ia beroperasi, beliau menjelaskan bahawa pesawat itu sudah keluar daripada ''routine maintainance check'' iaitu tempoh pemeriksaan selama 500 jam dan mempunyai jadual penyelenggaran.

"Selepas tamat tempoh terbang 500 jam, pesawat ini akan masuk untuk ''minor servicing'' (penyenggeraan kecil) di mana ia perlu dibuat ''functional check flight'' (penerbangan pemeriksaan fungsian) sebelum jurutebang berkelayakan menerbangkan pesawat itu untuk sesi penerbangan seterusnya," katanya.

Affendi berkata pesawat itu telah membuat penerbangan pertama pada Selasa lepas dan TUDM mendapati ia mengalami sedikit kerosakan dan telah dibaiki serta diterbangkan semula pada Rabu lepas.

"Waktu pesawat itu diterbangkan pada Rabu lepas, ia diisytiharkan sebagai pesawat yang telah menjalani pemeriksaan baik dan memenuhi aspek teknikal penerbangan," katanya. - BERNAMA

H: Difahamkan pilot tgh buat flying check flight time tu. Depa naik 45k feet. N turun menjunam ke 20k feet. Very high speed.
